The Dosing Summary Report By Carrier lists patients grouped by carrier with key information about the doses and type for their active orders. The report can be run several ways:
For any range of dates
By Carrier - for one specific carrier or all carriers
By Program - for one specific program or all programs
The headings of this report are:
These columns will tell users:
ID: The SMART Patient ID number.
Name: The first and last name of the patient.
DOB: The patient’s date of birth. It will be formatted in MM/DD/YYYY.
Recipient #: This is the RecipientID which can be found on the Patient’s Profile just under their chart number on the right hand side of the screen.
MIS #: The Medical Information System number assigned to the patient. This is set on the Patient Information Management screen under the Demographics tab.
Medication Type: This column shows the medication dispensed during the time period the report was run.
#of Doses - Disp: This column displays the number of doses that the patient received in the dispensary for the time period the report was run.
#of Doses - Tkhm: This column displays the number of doses that the patient took home for the time period the report was run. This column counts doses where the Takehome flag is checked. The exception is doses with dose type id of ‘V' (Travel) which is shown in it’s own column.
#of Doses - Total: This is the sum of the above two columns (dispensary doses plus takehome doses) for the time period that the report was run.
AWOL: The number of days the patient was AWOL during the time period of the report. This column counts doses with a dose type id of 'A'.
Travel: The number of days the patient was on a travel schedule for the time period of the report. This column counts doses with a dose type id of 'V'.
Susp: The number of days that a patient was suspended during the time period of the report. This column counts doses with a dose type id of 'U'.
Other: This column is used to count any other dose types that are not already covered by the other columns. This would show the total number of days that a patient had an alternate dose type for the time period the report was run. Details on this can be found in the Patient Inquiry screen by reviewing the Dose History tab on the bottom.
You can find this report from the Reports module (print icon) in SMART by selecting Medicating → Activity or Medicating → Summaries or Third Party → Services and then the report title itself.
