The Patient Contact Report shows whether a patient has had a clinical contact or in person dosing event in a specific date range.
The report is run for a date range.
The report can be run for a specific Program or all Programs. Programs appear alphabetically. Each new program will start on a new page.
The report can be run for a specific Insurance Carrier or All. When running for All, Insurance will appear alphabetically. Self pay patients will be listed under Cash.
The report can be run for a specific Counselor or All.
The report is Group by
For each counselor report lists
Patient ID and Name
Level of Care Assigned to patient
Coverage From Date and Thru Date if applicable
Whether patient received Group, Individual or Physical Dosing Event
Total patients listed
Report Column Definitions
Patient’s display name
Level of Care
The patient’s level of care assigned.

You can find this report from the Reports module (print icon) in SMART by selecting Management→ Clinical →and then the report title itself.
