The Direct Services Analysis Report is a way for Clinic Directors, Counselors, and other staff to evaluate if the provided hours of direct services (time spent with patients) is meeting the standard of care set by clinic administration and state requirements. It’s main purpose is to be run for a time period to evaluate if required direct hours are being satisfied by staff members. This report can be run:
For any range of dates
By Counselor - for one specific counselor or for all
By Program - for one specific program or all programs
The headings of this report are:
These columns will tell users:
ID #: The SMART Patient ID number.
Chart #: The SMART Patient’s Chart number.
Name: The first and last name of the patient.
Admit Date: The date that the patient was admitted formatted in MM/DD/YY.
Discharge Date: The date that the patient was discharged (if applicable) formatted in MM/DD/YY.
Last Seen: The date the patient last came to the clinic for a required service formatted in MM/DD/YY.
Last Group: The date of the patient’s last group session formatted in MM/DD/YY.
Level of Care: The patient’s assigned Level of Care as set during admission.
Hours - Req’d: How many direct hours this patient is required to receive for the time period of the report.
Hours - Provided: How many direct hours have been provided to the patient for the time period of the report.
Under: The number of hours that the patient still needs to be seen for direct services during the time period of the report.
Over: The extra direct hours that have been provided through services for the patient during the time period of the report.
% of Req’d: The Provided Direct Hours divided by The Required Direct Hours shown as a percentage.
You can find this report from the Reports module (print icon) in SMART by selecting Clinical → Quality or Management → Clinical and then the report title itself.
