By default, SMART will print the patient’s name, date of birth, chart number and identification number on any Release Authorization for that patient. A clinic can also configure the Release Settings to include additional patient-identifying information. These settings are available to users with access to the Administration module (Gears) in SMART through the Clinic Settings and on the Release Auth tab.
Select the Setting for Report Header and check the additional options you want to print on the Release Authorizations and be sure to choose Save Release Auth before closing this window:
With the Release Auth settings configured this way, any new release for a patient will be created with these defaulted to “checked” (as shown below) but should the patient decline to release that information, the staff member completing the release can uncheck that information to exclude it from printing on the paper release.
When any of these settings is turned on, this data will print on the top of the release as shown:
Changes made to these settings only impact Release Authorizations created after the changes have been saved. Releases that have been drafted or executed prior to the changes will retain the original patient identifiers.
This feature was released in SMART version 4.5.8.
