Updates to NY PAS Forms

Overview

SMART has made updates required by New York State’s Office of Addiction Services and Supports (OASAS) to some NY PAS forms, effective January 1st, 2024. This reference document outlines changes to the NY PAS 45, 44, and 26 forms.

NY PAS 45 Discharge Form

The following table outlines changes to the NY PAS 45 Discharge form and other related areas of the SMART interface.
